Kindergarten Roundup at Jackson Elementary

Kindergarten Roundup Registration Form

Format:  Students and parents will come to the Jackson Elementary commons.  The students will then go with the kindergarten teachers, Mrs. Curry & Mrs. Todd.  Students will be doing some fun activities during this time, having a snack and playing on the playground (if the weather cooperates).  With your permission, an audiologist and a speech pathologist will do a screening with your child.

Parents will meet during this time with Mr. Hayes (principal), Mrs. Herfel (school nurse), and Mrs. Anderson (counselor) for a short meeting. Then parents will be given time to complete the paperwork needed to enroll their child in kindergarten.  If parents finish before the scheduled ending time, they may leave and return for their student at the scheduled time.

FYI:  Public schools are required by state law to have several things on file before a child may enter school.  These are the requirements:

1. an official birth certificate from the state where your child was born, not the birth certificate from the hospital
2. current immunization records (including kindergarten shots)
3. Physical Exam (dated within six months of the start of kindergarten)
4. Eye Exam (dated within six months of the start of kindergarten)

Please bring the child's birth certificate and immunization records to Kindergarten Roundup to make copies (the original documents will be returned to you).  Also, remember to schedule your child's Physical Exam and Eye Exam.  Throughout the summer, Exam reports & updated immunization records may be turned into the Ponca School office.  Beginning August 1^st, you may turn them in at Jackson Elementary between 8 am and 3 pm.  You may also mail the records to Jackson Elementary, P.O. Box 67, Jackson, NE  68743.  All of the above documents must be received by August 6^th, or your child will not be allowed to attend kindergarten this fall.
